Insurance Information
We accept most major Insurance Plans. We do NOT accept Medicaid, Third Party Billing or Workman's Compensation.

Appointments
Please call our office to schedule an appointment. If you are unable to keep your appointment and to avoid an office visit charge, we require a 24-hour cancellation notice. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergency trauma call or emergency surgeries.

Fees & Payments
Payment is required for all office services at the time they are rendered unless prior arrangements have been made. Currently we accept cash, checks, VISA, MasterCard and debits cards. If we are a participating provider of your insurance company, we will file your claim for you. Do keep in mind that ultimately payment is the patient's responsibility.

Prescription Refill Requests
Please contact your pharmacy and ask them to fax (512-617-5598) a prescription refill request to our office. At that time, your doctor will approve or deny the request.
